Requirements: Bachelor Degree with major course work in psychology, social work, socialogy, education, or related field as agreed to by management and the union. Valid Pa. license and automobile.

This position provide the exciting opportunity of providing support coordination services for consumers. Some of the essential Job Functions for this position would include to Conduct Individual Support Plan process, Process ASSIST documents, Complete voter registration form, Conduct Pennhurst monitorings, Process OBRA forms, Complete PUNS process, Complete Personal Plan Information (PPI), and Complete Service Preference process. Minimum requirements include - Bachelor Degree with major course work in psychology,social work, sociology, vocational rehabilitation or related field. This person must possess a valid PA driver's license and an automobile. Excellent Benefit Package!
