Office of Long Term Living (OLTL) Service Coordination will assist individuals who receive OLTL services in gaining access to waiver services and other State Medicaid Plan services.  OLTL Service Coordination will also help access medical, social, and other services regardless of the funding source.  Service Coordination is the process of working with consumers whenever possible to identify, coordinate, and facilitate all necessary services.

What Happens During OLTL Service Coordination?

The Service Coordinator (SC) works to develop a partnership of respect, support, and collaboration with the person served and families. Within this partnership, the Service Coordinators develop an individual service plan (ISP) and help consumers and families. The ISP is based on the consumer's strengths and interests and which identifies specific objectives to reach the consumer's personal goals.

OLTL Service Coordination can include any of the following services:

  • Assistance to link to chosen treatment, other services, natural supports, community resources, and opportunities that are specified in the consumer's individual service plan;
  • Help addressing basic physical needs and supports that are necessary for a stable, safe, and healthy community life;
  • Connecting the individual and family to kindred, peer, and other social support networks that are important to the persons served.
  • Preventing and managing crisis by coordinating and/or assisting with crisis intervention and stabilization services as appropriate.
  • Working with consumers to ensure that their voice is heard when dealing with other service agencies, physicians, medical plans, and more. Over time, SCs work to help their consumers become effective advocates for themselves.
  • Monitoring the individual service plans and make sure treatments remain appropriate. SCs help consumers determine whether their service plan is not only meeting their needs, but is also working toward their hopes in life.
  • Coordinating a consumer's physical and mental health treatments, determine the best methods of finding funding for treatment, help find transportation to appointments, and more.
  • Completing necessary/appropriate documentation, including maintaining individual's eligibility for waiver services, copies of the Individual Service Plan (ISP) and service plan addendums, financial data and related information.

Your SC is mobile and can offer outreach in whatever setting the consumer prefers – home, community, or the SAM office – and allows for the best interaction with the consumer.

SCs can adjust the intensity and frequency of services provided according to the needs of the individual as identified in his/her individual service plan, particularly during times of transition or crisis. The SC will also review the level of care needs of the consumer periodically to ensure that needs are being met.

Service Coordination Services can be provided to individuals eligible for the following:

  • Aging Waiver
  • Attendant Care/ ACT 150 Waiver
  • Independence Waiver
  • OBRA Waiver
  • COMMCARE Waiver
