Mental Health Case Management Services
Case management is provided with the objective of coordinating mental health services, advocating on behalf of the consumer, and monitoring the consumer's various community services.

Types of Mental Health Case ManagementIntensive Case Management (ICM) and Resource Coordination (RC) ICM and RC services are to designed to coordinate mental health services, assist in identifying community resources, and to support consumers in establishing personal goals. Intake staff will determine the appropriate level of case management. Consumers of RC meet with their case manager at least once every two months while ICM consumers meet with their case manager at least once every two weeks.
Blended Case ManagementBlended Case Management is the same service but allows case managers additional flexibility to provide more intensive or less intensive case management services without having to transfer consumers to another case manager.
