Adams County

Intake

Residents of York and Adams Counties should contact the office for an appointment which will be scheduled based on the availability of the consumer but generally Monday through Friday between 8:00 AM and 4:30 PM. Call 888-245-8744 for an appointment

Mental Health Case Management Services

Case management is provided with the objective of coordinating mental health services, advocating on behalf of the consumer, and monitoring the consumer's various community services.

Types of Mental Health Case Management

Intensive Case Management (ICM) and Resource Coordination (RC)

The objectives of ICM and RC are to coordinate mental health services, assist in identifying community resources, and to support consumers in establishing personal goals. Intake staff will determine the apporpriate level of case management. Consumers of RC meet with their case manager at least once every two months while ICM consumers meet with their case manager at least once every two weeks.
